The Aging and Disability Resource Center (ADRC) is a single point of entry for older adults and people with disabilities who need information and assistance with community resources and access to publicly funded, community-based long-term care programs. The Information & Assistance Specialist will provide information and assistance over the phone, in person, and via email. Duties and Responsibilities:

  • Provide prompt, accurate, unbiased, and friendly responses to requests for information about an array of topics and for access to long-term care via email, over the phone, and in person.
  • Provide follow-up connections to clients following Wisconsin Department of Health Services guidelines.
  • Maintain up to date knowledge of community resource benefit programs, Options Counseling procedures, and program guidelines. 
  • Obtain and maintain Inform USA certification within 1 year of employment.
  • Maintain up to date knowledge of eligibility, enrollment & disenrollment procedures and protocols for Family Care, IRIS, Partnership and PACE Programs.
  • Provide advocacy and assist people to obtain needed services.
  • Document contacts, make referrals to programs within the ADRC as well as outside organizations, and participate in time reporting applying State of Wisconsin DHS guidelines.
  • Attend and participate in staff development programs, in-service training, staff meetings, professional seminars, and outreach events.
  • Obtain Options Counseling certification after the first month of employment.
  • Maintain a high level of confidentiality and performance that is established in ADRC contract, quality assurance plans and agency standards. 
Qualifications:

Qualifications:

  • Bachelor’s degree in human service-related field
  • The equivalent of at least one year’s full-time experience in a health or human service field, working with one or more of the client populations served by the ADRC. Qualifying work experience may be paid or unpaid, including internships and field placements.
  • Excellent active listening, interviewing, and advocacy skills 
  • Knowledge of case work principles, long-term care, social service delivery systems and community resources
  • Knowledge of Medicaid, Medicare and other public benefit programs preferred 
  • Ability to organize and plan work effectively, and independently
  • Ability to communicate clearly and effectively, orally and in writing
  • Certification for Community Resource Specialist-Aging/Disabilities preferred
  • Proficient use of computers, multifunctional devices, and multi-line phone.
