We are looking for a trusted peer from the medical community—someone who already has the ears of discharge planners and physicians—to help bridge the gap between hospital care and a safe home environment.

What You’ll Do (Responsibilities):

• Peer-to-Peer Outreach: Visit Santa Monica hospitals, clinics, and discharge planners to educate them on how our "Cared-4℠" program supports their patients.
• Relationship Management: Maintain and grow your existing network of physicians, case managers, and social workers.
• Clinical Collaboration: Meet with families and clinical teams to ensure a "warm handoff" from the hospital to our care team.
• Community Presence: Represent us at Westside networking events or health fairs that align with your expertise.

What We’re Looking For (Qualifications):

• Medical Background: You are a Nurse (RN/LVN), Social Worker (MSW), or have a strong clinical/medical background.
• The "Santa Monica Connection": You have established, existing relationships with referral sources (hospitals, discharge planners, or senior living) in the Santa Monica/West LA area.
• Excellent Communicator: You can explain the "why" behind home care with empathy and professional authority.
• Independent: You are self-motivated and comfortable managing your own schedule and outreach log.

Mobile: Must have a valid driver’s license and reliable transportation for local visits.

• Pay: $40.00 - $45.00 per hour.
• Job Type: Part-time (Flexible hours).
• Location: Santa Monica and surrounding Westside neighborhoods.
