CURE HOME HEALTH INC is a Medicare-certified home health agency serving Los Angeles County. We provide skilled nursing and therapy services with fast admissions, reliable communication, and strong clinical oversight.
We are growing and building a consistent hospital and SNF referral network. We are looking for someone who knows how to build relationships and drive admissions — not someone who drops brochures.
Position OverviewThis is a field-based liaison role focused on developing and maintaining referral relationships with:
- Hospitals
- Skilled Nursing Facilities (SNFs)
- Case Managers
- Discharge Planners
- Assisted Living Facilities
Your primary responsibility is to grow census through active outreach, relationship management, and coordination with our intake and clinical teams.
This is a performance-driven role with strong earning potential.
Key Responsibilities- Develop and manage hospital and SNF referral relationships
- Conduct regular in-person visits to referral sources
- Secure and track referral volume
- Coordinate timely admissions with intake team
- Communicate SOC timelines clearly and confidently
- Follow up with referral sources after admission
- Maintain referral tracking reports
- Represent the agency professionally in the field
- 2 years experience in home health, hospice, rehab, or post-acute referral development preferred
- Strong relationships within Los Angeles healthcare facilities highly preferred
- Proven ability to grow census
- Excellent communication and follow-up skills
- Self-motivated and comfortable working independently in the field
