Position Responsibilities:
- Provide personal care and assistance with daily living activities
- Support patients with mobility, hygiene, and companionship
- Follow individualized care plans
- Maintain accurate documentation and communication with the care team
Minimum Requirements:
- Valid HHA Certificate or CNA License
- Current CPR Certification
- Level II Background Screening (or ability to obtain)
- Current TB Screening and Physical
- Valid Driver’s License
- Reliable vehicle with auto insurance
