This role is ideal for a Physician Assistant (PA-C) seeking work-life balance, autonomy, and a deeper patient connection. You'll round at various senior communities and play a vital role in delivering coordinated, relationship-based care.
Responsibilities:
- Visit patients in assisted living and memory care settings
- Provide physical exams, manage ongoing conditions, and adjust care plans
- Collaborate with physicians and support staff to optimize treatment
- Educate families and facility teams on care coordination
Qualifications:
- MN-licensed PA in good standing
- Experience in senior care, home visits, or family medicine
- Excellent documentation and interpersonal skills
- Valid driver’s license and vehicle required
