One RN. A Lot of Different Nursing.
Your visits may include:
- Comprehensive nursing assessments
- Wound care and other ordered treatments
- Medication reconciliation, management, and education
- Post-hospitalization nursing
- Chronic disease management
- Patient and caregiver education
- Monitoring medically complex conditions
- Recognizing and communicating changes in condition
- Admissions and additional skilled visits as needed
- Collaboration with RN Case Managers and physicians
- Coordination with PTs, OTs, SLPs, social workers, and other clinicians
- Accurate and timely clinical documentation
- Participation in an on-call and weekend rotation
Bring the Skills You've Already Built
We're interested in RNs from Home Health, Med/Surg, Acute Care, TCU, Skilled Nursing, Rehabilitation, Hospice, Post-Acute Care, and other hands-on clinical environments.
You'll need an active Minnesota Registered Nurse (RN) license, strong assessment and critical-thinking skills, confidence working independently, excellent communication, and a valid driver's license with reliable transportation.
Home health, wound care, chronic disease management, and OASIS experience are preferred.
