To Stand Out
Email Elena Ehrlich at elena.ehrlich@hhcare.net with your resume and tell us about your nursing background, the types of skilled nursing care you enjoy most, and what you think patients struggle with most once they return home.
The Discharge Instructions Say What To Do. You Help Patients Figure Out How To Do It.
Take these medications.
Check your blood sugar.
Change the dressing.
Monitor your symptoms.
Follow this diet.
Call the doctor if anything changes.
It can all make perfect sense when a patient leaves the hospital.
Then they get home.
That's where an experienced Visit Nurse can make all the difference.
Home Health Care, Inc. is hiring a Registered Nurse (RN) Visit Nurse to provide skilled nursing throughout the western Twin Cities Metro and surrounding communities.
You'll help patients take everything they've been told and actually make it work in their everyday lives.
Turn Clinical Instructions Into Confident Patients
Home health lets you see something nurses in other settings often don't: what happens after the patient walks out the door.
You see the medications they're actually taking. The food in their kitchen. The caregiver helping with their wound. The stairs they're navigating. The equipment they're trying to use.
And suddenly, patient education becomes a lot more than handing someone instructions.
As a Visit Nurse, you'll provide hands-on skilled nursing while helping patients and caregivers better understand their conditions, treatments, medications, and recovery.
Why This Opportunity Stands Out

You'll Do More Than Tell Patients What They Should Do
You'll help them understand why it matters.
Maybe a patient isn't taking a medication because they don't understand its purpose.
Maybe a caregiver isn't comfortable with wound care.
Maybe someone with CHF doesn't understand why daily weights matter.
Maybe a diabetic patient nods through the education but still doesn't feel confident managing their condition.
Great nursing isn't simply delivering information.
It's making sure the patient can use it.
That's an important part of what you'll do every day.

This role gives you the opportunity to take those skills into the patient's environment and understand what they're really dealing with between appointments.

- Excellent patient and caregiver education skills
- Confidence providing hands-on skilled nursing care
- Ability to work independently in patient homes

Great Patient Education Doesn't End With "Do You Understand?"
It ends when the patient can confidently tell you what they're going to do after you leave.
That's the difference an experienced Visit Nurse can make.
You'll provide skilled nursing throughout Mound, Maple Plain, Independence, Howard Lake, Delano, Montrose, Watertown, Mayer, Waverly, Wayzata, Excelsior, Cokato, Orono, Victoria, Cologne, Carver, St. Bonifacius, Minnetonka, and surrounding communities, helping patients turn complicated healthcare instructions into something they can actually manage at home.
You'll have $75,000-$100,000 annual compensation, a $4,000 First-Year Bonus, mileage reimbursement, comprehensive benefits, and an experienced clinical team supporting you.
If you're an RN who knows great nursing isn't just what you do for a patient, but what you teach them to do for themselves, we'd like to meet you.
