Overview
We are seeking a dedicated and compassionate PRN RN for our Home Health Care. This role involves providing high-quality nursing care to patients in their homes, ensuring their comfort and well-being. Duties
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed, including IV infusion and injections.
- Provide wound care, catheter care, and assist with activities of daily living.
- Monitor vital signs and report any changes in patient conditions to the healthcare team.
- Educate patients and families on health management, medication administration, and self-care techniques.
- Collaborate with interdisciplinary teams to ensure coordinated patient care.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Utilize electronic medical record (EMR) systems for documentation and patient tracking.
- Perform triage assessments for urgent situations and coordinate emergency interventions when necessary.
- Support patients with chronic conditions such as diabetes, cardiac issues, or respiratory diseases.
Experience
Candidates should have a robust background in nursing with experience in home health or similar settings. Preferred qualifications include:
- Proficiency with EMR systems
- Experience in critical care environments
- Familiarity with various nursing specialties including geriatrics, pediatrics, hospice & palliative medicine, and behavioral health.
- Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), and case management principles.
- Previous experience working with individuals with developmental disabilities or memory care needs is advantageous.

Pay: $28.00 - $47.54 per hour
