Overview
We are seeking a compassionate and dedicated PRN Licensed Practical Nurse (LPN) to join our Home Health Care team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings. As an LPN, you will play a crucial role in supporting registered nurses and physicians while ensuring the comfort and well-being of patients.
Duties
- Provide direct patient care, including administering medications and treatments as prescribed.
- Conduct patient assessments and monitor vital signs.
- Assist with medical procedures and perform basic life support as needed.
- Document patient information accurately in electronic health records (EHR) systems.
- Collaborate with healthcare team members to develop and implement individualized care plans.
- Educate patients and families on health management and disease prevention.
- Maintain a clean and safe environment for patients.
- Participate in discharge planning and case management activities.
- Support patients with activities of daily living (ADLs) such as bathing, dressing, and feeding.
Skills
Candidates should possess a combination of required and preferred skills, including but not limited to:
- Proficiency in using EMR systems such as Epic, Cerner, or Athenahealth.
- Experience in critical care, long-term care, assisted living, or home care settings.
- Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), and CPT coding.
- Familiarity with acute pain management, geriatrics, dementia care, and Alzheimer’s care.
- Skills in IV infusion, venipuncture, catheterization, medication administration, and specimen collection/processing.
- Ability to perform physical examinations and triage patients effectively.
- Experience working with individuals with developmental disabilities or in hospice care.
- Strong understanding of infection control practices and aseptic techniques.

Pay: $24.00 - $41.45 per hour
