You'll take the clinical skills you've developed as an RN and bring them directly to patients who need them.

Your Nursing Skills Belong in the Community, Too

Another needs help understanding several new medications.
Your next patient may be managing CHF, COPD, diabetes, or another chronic condition.
Later, you're supporting someone who recently returned home from the hospital and isn't recovering quite the way everyone expected.

What You'll Be Doing
- Provide skilled nursing visits throughout your assigned territory
- Complete nursing assessments and ongoing evaluations
- Perform wound care and other ordered skilled treatments
- Provide medication management and patient education
- Monitor chronic and medically complex conditions
- Support patients following hospitalization, surgery, illness, or injury
- Recognize changes in condition and respond appropriately
- Educate patients, families, and caregivers
- Support admissions and additional skilled visits as needed
- Complete accurate and timely clinical documentation
- Communicate findings and concerns to RN Case Managers and physicians
- Collaborate with PTs, OTs, SLPs, social workers, and other clinicians
- Participate in an on-call and weekend rotation
Bring the Experience You've Already Built
We're interested in RNs from Home Health, Med/Surg, Acute Care, TCU, Skilled Nursing, Rehabilitation, Hospice, Post-Acute Care, and other hands-on nursing environments.
You don't need to leave your clinical experience behind to move into home health.
You'll use your assessment skills, critical thinking, patient education, wound care experience, medication knowledge, and ability to recognize changes in condition.
The biggest difference is where you're using them.
What You Bring
Required:
- Active Minnesota Registered Nurse (RN) license
- Strong clinical assessment and critical-thinking skills
- Excellent patient and caregiver communication
- Confidence providing hands-on skilled nursing care
- Ability to work independently in patient homes
- Strong organization and time-management skills
- Valid driver's license and reliable transportation
Preferred:
- Home health experience
- Hospice, TCU, or post-acute experience
- Wound care or chronic disease management experience
- OASIS experience
