You'll assess within your LPN scope, communicate what you're seeing, and help the clinical team respond appropriately.
What You'll Be Doing
- Complete skilled nursing visits in patient homes
- Perform ongoing nursing assessments within LPN scope
- Administer medications and treatments according to physician orders
- Provide wound care and other ordered skilled treatments
- Monitor chronic and medically complex conditions
- Observe patients for changes in condition
- Reinforce medication and disease-management education
- Educate patients, families, and caregivers
- Communicate clinical concerns to RN Case Managers
- Support individualized plans of care
- Complete accurate and timely documentation
- Collaborate with the interdisciplinary care team
- Help patients recover and remain safely at home
- Participate in required on-call and weekend rotations

What You Bring
Required:
- Active Minnesota Licensed Practical Nurse (LPN) license
- Minimum one year of clinical nursing experience
- Strong nursing observation and assessment skills
- Excellent patient and caregiver communication
- Ability to recognize and report changes in condition
- Confidence working independently in patient homes
- Strong organization and time-management skills
- Valid driver's license and reliable transportation
- Willingness to travel throughout your assigned territory
Preferred:
- Home health experience
- Hospice experience
- TCU, skilled nursing, or long-term care experience
- Wound care or chronic disease management experience
