Primary Responsibilities:
- Perform comprehensive Start of Care nursing assessments
- Complete high-quality OASIS documentation with accuracy and timeliness
- Develop individualized care plans based on patient goals, safety needs, and medical conditions
- Educate patients and their families on expectations, medication management, and resources
- Partner with therapists, case managers, and providers to ensure seamless care coordination
- Serve as a clinical leader who shapes the patient’s home health experience from day one
Required Qualifications:
- Active RN license in Minnesota
- Strong experience with OASIS and SOC assessments
- Background in home health or community-based nursing preferred
- Reliable vehicle and valid driver’s license
