What You'll Be DoingTransform Communication And Swallowing Function
- Deliver one-on-one speech, language, cognitive, and swallowing therapy
- Perform comprehensive evaluations and develop individualized treatment plans
- Provide functional interventions tailored to communication, cognition, and feeding
- Educate patients and families on strategies that reinforce progress at home
- Help patients improve safety, independence, and quality of life
Collaborate With An Exceptional Team
- Work closely with nurses, therapists, physicians, and interdisciplinary clinicians
- Document care accurately using EMR systems
- Monitor progress and adjust treatment plans accordingly
- Contribute to outstanding patient outcomes and continuity of care
Required Qualifications
- Active Minnesota Speech Language Pathologist license
- Valid driver's license and reliable transportation
- Strong communication and clinical reasoning skills
- Passion for patient-centered, relationship-based care
Preferred Qualifications
- Home health experience
- EMR experience
- Experience with medically complex or geriatric populations
- Interest in cognitive and swallowing disorders
