You'll have the backing, referrals, interdisciplinary team, and infrastructure of an established home health organization while having significant independence in how you manage your patients and your workday.
Build a Territory, Not Just a Schedule
Over time, you begin to know the communities you're serving. You develop relationships with patients and caregivers, collaborate with the clinicians working in your territory, and become someone the team trusts when a patient needs Physical Therapy.
This is still a clinical PT position. You'll evaluate, treat, document, coordinate care, and maintain productivity expectations. The difference is that you're doing it with greater ownership over your day and a defined geographic focus, rather than spending every day inside the same four walls.

Our service area includes communities throughout the North and Northeast Metro, but your schedule is built around patient locations and geographic efficiency. Rather than looking at this as 19 separate cities, think of it as several connected areas:

Additional patients may be located in nearby communities such as Centerville, Columbus, Harris, Scandia, Stacy, Taylors Falls, Marine on St. Croix, Shafer, Center City, and surrounding areas based on patient need and scheduling.
The goal isn't to send you zigzagging across the entire territory. It's to build a productive field schedule while meeting patient needs throughout the region.
Practice With More Clinical Ownership
Home health gives experienced PTs room to make decisions. You'll complete your evaluation, determine appropriate goals and interventions, monitor progress, communicate with the care team, and adjust your approach based on what you're seeing.
You'll still have an experienced clinical team behind you, but you won't have someone standing outside the treatment room directing every part of your day. We're looking for a PT whose experience has prepared them for that level of trust.
What You'll Be Doing
- Complete comprehensive Physical Therapy evaluations in patient homes
- Develop individualized plans of care and functional goals
- Provide skilled interventions addressing strength, balance, gait, transfers, mobility, and safety
- Identify fall risks and barriers to independence
- Recommend equipment and appropriate home modifications
- Educate patients, families, and caregivers
- Monitor progress and modify treatment plans as clinically appropriate
- Coordinate care with PTAs, nurses, physicians, OTs, and other disciplines
- Complete accurate and timely clinical documentation
- Manage a productive field-based schedule within your territory

We're interested in Physical Therapists from Outpatient Orthopedics, Acute Care, Inpatient Rehabilitation, Skilled Nursing, Transitional Care, Senior Living, and Home Health. What You Bring
- Active Minnesota Physical Therapist license
- PT or DPT
- Valid driver's license and reliable transportation
- Strong clinical assessment and decision-making skills
- Ability to independently manage a field-based schedule
- Strong organization and time-management skills
- Excellent patient and caregiver communication
- Home health experience preferred
- OASIS experience preferred
