What You'll Be Doing
• Provide on-site primary care and rounding services throughout assisted living, memory care, and long-term care communities.
• Perform comprehensive assessments, chronic disease management, and preventative care.
• Develop and adjust individualized treatment plans.
• Collaborate with nurses, therapists, caregivers, physicians, and facility leadership.
• Support transitions of care and help reduce avoidable emergency room visits and hospitalizations.
• Serve as a key clinical partner for patients, families, and community staff.
Qualifications
• Active Minnesota Physician Assistant (PA-C) license in good standing.
• Experience in geriatrics, family medicine, internal medicine, post-acute care, hospital medicine, or related settings preferred.
• Strong communication skills and confidence working independently.
• Valid driver's license and reliable transportation.
