Duties
- Provide comprehensive patient observation to monitor health status and detect changes requiring medical attention
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Administer medications accurately following prescribed medical instructions and document medication records diligently
- Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to update patient information securely and efficiently
- Perform specialized care such as catheter care, hospice support, dementia and Alzheimer’s care, memory care, and working with individuals with developmental disabilities
- Use medical terminology correctly when documenting patient conditions and communicating with healthcare teams
- Employ safety equipment like Hoyer lifts for patient transfers and utilize vital signs monitoring tools to assess health status
- Support patients with meal preparation, light cleaning, laundry, and maintaining a clean living environment in accordance with health standards
- Follow HIPAA regulations to ensure patient confidentiality and privacy at all times
- Collaborate with healthcare professionals to develop personalized care plans tailored to each patient's needs
Qualifications
- Proven experience in home health care or long-term care settings; nursing home experience is highly valued
- Strong knowledge of medical terminology, medication administration, and vital signs assessment
- Demonstrated ability to assist with ADLs and provide compassionate senior or disability care
- Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as hospice care or dementia training are a plus
- Experience working with individuals with disabilities or developmental challenges preferred
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Ability to lift patients safely using equipment like Hoyer lifts and perform patient transfers confidently
- Excellent communication skills to coordinate effectively with healthcare teams and family members
- Valid driver’s license preferred for travel between client locations; reliable transportation required
