Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
- Monitor and observe patients’ health status, noting changes in condition and reporting to healthcare teams as needed.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking.
- Administer medications accurately following prescribed care plans while maintaining compliance with HIPAA regulations to ensure privacy and confidentiality.
- Use specialized equipment such as Hoyer lifts or resident’s personal assistive devices to safely transfer clients with mobility challenges.
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces.
- Document care activities thoroughly in accordance with agency protocols and participate in ongoing training related to patient care, dementia, Alzheimer's, autism, or hospice needs.
Qualifications
- Proven experience in home health, assisted living, nursing home care, or group home settings is highly desirable.
- Knowledge of patient observation techniques, vital signs monitoring, medication administration, and care planning.
- Certifications such as CPR and First Aid are required; additional training in dementia or Alzheimer's care is a plus.
- Ability to lift heavy objects safely and perform physically demanding tasks while maintaining compassion and patience.
- Familiarity with HIPAA regulations to protect client confidentiality at all times.
- Experience working with individuals with developmental disabilities or autism is preferred.
