What You Will Own
As Director of Clinical Services, you will be responsible for the full clinical oversight of the agency, including:
Clinical & Regulatory Leadership
- Oversight of all skilled and non-skilled clinical staff
- Ensure full compliance with Medicare Conditions of Participation
- Maintain survey readiness at all times
- Oversee OASIS accuracy, timeliness, and quality
- Monitor documentation integrity and billing compliance
- Ensure PDGM optimization through strong clinical coordination
Operational Oversight
- Develop and manage staffing plans
- Supervise clinical managers and field clinicians
- Review admissions, recertifications, and discharges
- Participate in hiring, onboarding, and competency validation
- Lead case conferences and interdisciplinary coordination
Quality & Performance Management
- Implement QA/PI initiatives
- Monitor hospitalization rates and outcomes
- Provide ongoing clinical education and corrective guidance
- Strengthen accountability culture within the team
Clinical Support
- Provide direct patient care when clinically necessary
- Act as agency clinical authority
Ideal Candidate Profile
We are specifically seeking a strong Medicare Home Health leader with:
- Active RN License (Connecticut)
- Minimum 2 years Medicare-certified Home Health experience
- At least 1 year in a supervisory/clinical management role
- Demonstrated OASIS proficiency
- Strong knowledge of:
- Medicare Conditions of Participation
- PDGM
- State regulatory standards
- Survey preparedness
- Β
- WellSky (Kinnser) experience preferred
- Ability to lead confidently and hold staff accountable
Required Compliance:
- Current CPR
- TB screening (within 12 months)
- Physical exam within 6 months prior to patient contact
- Valid driverβs license & automobile insurance
