Home Care Coordinator

Job description

A Home Care Coordinator executes and supports each clients’ home care solution.

This includes maintenance of ongoing communication and coordination with the

referral, family members, client, healthcare providers and intake sources for all

clients. The Home Care Coordinator shall plan for all home healthcare service

coverage from therapy, nursing, caregivers, and companions, while ensuring that

the patient/family are receiving excellent customer service.

REQUIREMENTS:

High school degree or equivalent required

Bilingual required

Proven experience in healthcare/home care coordination, customer service and or

staffing

Working knowledge of HHA Exchange is a plus

Ability to communicate effectively with employee, patients and their families

Ability to define problems and tasks, collect data and establish facts, take action

and resolve issues!

Intake Coordinator

Job description

Intake Coordinators are responsible for processing referrals received from marketing sources such as hospitals, nursing homes, and community physicians. Coordinators contact each patient to educate them on the services we can provide and assist them with their homecare needs for skilled and non-skilled services. In this important position Intake Coordinators accept the great responsibility of following a patient from the date of discharge through enrollment for long term care, keeping the patient and family members informed at all times to ensure all patients have caring and professional experience with our Multiple Home Care Partners.

JOB RESPONSIBILITIES INCLUDE:

Receive referrals from referral sources

Verify patient’s insurance to determine eligibility

Educate patients on services we provide

Send referral to MLTC for potential enrollment or Medicaid assistance

Send referral to appropriate CHHA for skilled services

Determine what services were approved by insurance

Ensure agency receives authorization for MLTC cases and provide case specifics to agency's HHA Coordination

Payroll Coordinator

Job description

Responsibilities:- Process and manage payroll for employees, ensuring accuracy and timeliness- Prepare and distribute payroll reports to management- Maintain employee records and ensure compliance with company policies and procedures- Reconcile payroll discrepancies and resolve any issues or discrepancies- Prepare and submit payroll tax filings and other required reports- Assist with the implementation and maintenance of payroll systems (e.g., Kronos, Workday)- Collaborate with HR to ensure accurate employee data and proper onboarding/offboarding procedures- Provide support for payroll-related inquiries from employees
