Role Description
This is a full-time, on-site Scheduling Care Coordinator role based in Oak Brook, IL. The Scheduling Care Coordinator will manage the daily scheduling of home care services, ensuring that client needs are met with accuracy and efficiency. Key responsibilities include maintaining client schedules, coordinating care provider assignments, updating service records, and communicating with clients, families, and care teams to resolve scheduling conflicts. Candidates will also be expected to maintain a high level of client service and ensure smooth operations in alignment with company standards.
Qualifications
- Proven experience in Care Coordination and Case Management to manage schedules, assignments, and care plans effectively
- Strong Communication and Customer Service skills to interact with clients, families, and team members professionally and empathetically
- Familiarity with Home Care operations and procedures
- Proficiency in organizational and time management skills with attention to detail
- Ability to handle conflict resolution and problem-solving efficiently
- Competency in relevant computer systems and scheduling software
- Experience in a healthcare or home care setting is a plus
- High school diploma or equivalent required; additional certifications in related fields are a bonus
