The Care Coordination Outreach Specialist is a connector between vulnerable community members and the services designed to support their health, stability, and independence. This role supports Washington State’s Health Home Program and the NSACH Community Hub, with a strong focus on outreach, engagement, enrollment, and early client support.

The Outreach Specialist is responsible for conducting a high volume of outbound calls, connecting with referred clients, building trust quickly, and successfully enrolling individuals into care coordination services. This role plays a critical part in ensuring community members are connected to services that improve health outcomes and overall well-being.

In addition to outreach and enrollment, this position carries a small caseload and provides short-term, stabilization support to newly enrolled clients. This includes conducting face-to-face and/ home visits as needed and monitoring care plan effectiveness to ensure a successful transition into ongoing care coordination services.

We are seeking someone who is confident on the phone, persistent, and motivated by results. The ideal candidate is comfortable with high daily outreach, can navigate resistance, and clearly communicates the value of services in a way that encourages participation.

 

Responsibilities

Health Home Program Outreach (50%)

  • Conduct high-volume outbound calls to Medicaid member referrals to introduce the Health Home Program and confirm eligibility
  • Clearly explain program benefits using trauma-informed, culturally responsive communication
  • Obtain verbal consent for enrollment and schedule initial visits with Care Coordinators
  • Provide short-term support to newly enrolled clients, including conducting face-to-face/home visits as needed
  • Monitor early care plan effectiveness and support client engagement during transition to ongoing services
  • Accurately document outreach efforts, enrollment status, and client interactions in the electronic health record system
  • Collaborate closely with Care Coordinators to ensure seamless handoffs and continuity of care

 

Community Hub Outreach (50%)

  • Conduct outreach to individuals referred through the NSACH Community Hub Program in alignment with partner agency protocols
  • Build trust with clients and support navigation to services such as housing, food access, transportation, and healthcare
  • Work collaboratively with community-based organizations and internal teams to address barriers to care
  • Maintain accurate tracking and documentation of outreach activities and outcomes

Qualifications:

Minimum Qualifications

  • High school diploma or equivalent
  • One year of experience in outreach, enrollment, case management, or health navigation
  • Familiarity with Medicaid/Medicare programs and Dual Special Needs populations
  • Ability to work independently and manage time across multiple programs
  • Strong written and verbal communication skills
  • Knowledge of healthcare systems and community-based resources
  • Strong organizational skills and ability to prioritize in a fast-paced environment
  • Proficiency with Microsoft Office and comfort learning EHR and data systems

Preferred Qualifications:

  • Associate or bachelor’s degree in social work, public health, or a human services field
  • Knowledge and application of Trauma-Informed Care practices 
  • Valid WA State driver’s license and current automobile insurance 
  • Experience working with older adults and disabled adults
  • Experience engaging with diverse communities
  • Experience with Washington State’s Health Home or Community Hub models
  • Bilingual or multilingual in a language widely spoken in the region (e.g., Spanish, Russian, Somali, Vietnamese)

Work Environment and Physical Demands:

  • This work is typically performed either in an office setting, client home or community venues in Snohomish County
  • Functions are usually performed sitting but may spend some amount of time standing
  • When necessary, the ability to work in a semi-open office setting with frequent in-person interactions
  • Ability to use full range of senses as needed to complete the job's essential duties
  • Work requires occasional travel away from the office during normal business hours

SUMMARY OF BENEFITS

Employees working 25 hours or more are eligible to enroll in medical, dental, vision, basic life, and critical illness insurance, with an increased cost share for employees scheduled for 25-29 hours/week. Employees working 20 hours or more per week receive 11 designated paid holidays plus 2 "floating holidays" per year, as well as paid vacation and sick time. Employees age 21 and older are able to enroll in our 401K plan upon hire and, after one year and at least 1000 hours worked, will be eligible for our matching program.
