Position Overview: As a Social Worker, you’ll help plan, organize, and direct social services to meet the emotional and social needs of our residents. You’ll play a key role in ensuring each resident’s well-being and supporting their adjustment to life changes.

Qualifications:

• Active social work license in Massachusetts (LSW, LICSW, LSWA, or LCSW).

• Bachelor’s degree in social work from an accredited school.

• At least 1 year of experience in a hospital, skilled nursing facility, or related healthcare setting preferred.

Key Responsibilities:

• Coordinate and monitor resident care conferences.

• Support residents and families through transitions and crises (illness, disability, loss).

• Advocate for residents’ personal needs and utilize community resources.

• Complete comprehensive documentation promptly.

• Comply with privacy policies and protect resident health information.

• Support staff with resident-related issues.

• Perform other duties as assigned.
