LICENSED SOCIAL WORKER
Blaire House of Milford is a family-owned and operated skilled nursing center that has been serving seniors for nearly 50 years. Our 73-bed facility specializes in Short Term Care, Long Term Care, Rehabilitation, and Sub-acute Care.
Job Summary
- Develop a comprehensive social history.
- Providing social intervention, such as counseling, as patient advocate, as liaison with the community, or regarding financial or emotional issues.
- Maintaining social service progress notes on patient medical records which reflect any changes in patient psycho-social or physical status, evaluation of family interactions, identification of new problem areas, and review of social plans, goals and progress made.
- Develop initial discharge plan and document in clinical record, revising until discharge with available community services.
Qualifications:
- EDUCATION: B.A. or M.A. degree in social work or related field.
- TRAINING AND EXPERIENCE: At least one year direct social work experience in a long-term care facility. Must be a licensed social worker at the LSW level of licensure or higher. Must have a genuine interest in working with geriatric patients and a sincere concern for their welfare and happiness.
Hourly Rate: $35 - $39
