Position Summary

The Clinical Quality Coordinator for Outpatient Services is responsible for leading and supporting quality improvement initiatives, clinical documentation accuracy, and regulatory compliance across outpatient services. This role will work closely with the Family Medicine Clinics, Surgery Department, Outpatient Specialty Clinics, and Cardiac Rehab to ensure high standards of patient care, documentation integrity, and operational excellence.

Essential Job Duties

  • Clinical Quality Management
    • Develop, implement, and monitor quality improvement initiatives across outpatient departments.
    • Analyze clinical performance data to identify trends, gaps, and opportunities for improvement.
    • Collaborate with department leaders to develop action plans and track progress toward quality goals.
    • Ensure compliance with regulatory standards 
    • Lead root cause analyses and performance improvement projects.
  • Quality Measurement and Reporting
    • Coordinate collection, validation, and submission of clinical quality data for Rural Health Clinics (RHC) quality measures, value-based programs, and other regulatory reporting requirements.
    • Monitor performance on key clinical metrics including preventive care, chronic disease management, patient safety, and population health indicators.
    • Maintain dashboards and reports that track clinic performance against internal benchmarks and national standards.
    • Ensure accurate documentation and data capture within the Electronic Health Record (EHR) to support quality reporting.
  • Accountable Care Organization (ACO) Participation
    • Support clinic participation in Accountable Care Organization (ACO) programs by monitoring required quality measures and utilization metrics.
    • Collaborate with providers and care teams to improve performance on ACO quality measures, including preventive services, chronic disease outcomes, and patient experience.
    • Assist with data extraction, validation, and submission of ACO performance metrics.
    • Participate in ACO quality meetings and communicate updates, requirements, and performance results to clinic leadership and staff.
  • Quality Improvement (QI) Initiatives
    • Coordinate and support quality improvement projects focused on improving patient outcomes, workflow efficiency, and regulatory compliance.
    • Utilize recognized improvement methodologies such as Plan-Do-Study-Act (PDSA) cycles.
    • Facilitate interdisciplinary teams in identifying improvement opportunities and implementing corrective actions.
    • Track improvement initiatives and measure effectiveness through data analysis.
  • Data Analysis and Clinical Performance Monitoring
    • Analyze clinical data to identify trends, gaps in care, and opportunities for improvement.
    • Provide actionable insights to providers and clinic leadership through reports and presentations.
    • Work with IT/EHR teams to optimize workflows, templates, and reports that support quality data capture.
  • Provider and Staff Engagement
    • Educate clinical staff on quality measures, documentation requirements, and improvement initiatives.
    • Serve as a resource for providers regarding clinical quality metrics and performance expectations.
    • Provide feedback to care teams on performance trends and improvement opportunities.
  • Population Health and Care Gap Management
    • Assist with identification and closure of care gaps such as preventive screenings, immunizations, and chronic disease monitoring.
    • Collaborate with care managers, nurses, and providers to develop strategies to improve patient outcomes.
    • Support outreach initiatives to improve compliance with recommended care.
  • Collaboration and Communication
    • Work collaboratively with clinic leadership, providers, nursing staff, informatics, and administrative teams to advance quality goals.
    • Participate in organizational committees related to Quality Assurance and Performance Improvement (QAPI).
    • Present performance data and quality improvement progress to leadership and committees.
  • Documentation and Program Coordination
    • Maintain records of quality improvement activities, meeting minutes, and performance reports.
    • Coordinate quality program timelines, reporting deadlines, and deliverables.
    • Assist leadership in developing strategic quality goals aligned with organizational priorities.

Requirements

  • Education
    • Bachelor’s degree in Nursing, Healthcare Administration, Public Health, Health Information Management, Clinical Informatics or Allied Health Field required.
  • Licenses, Certifications, or Registrations
    • Valid Driver’s License, required.
    • RN, CPHQ, Lean Six Sigma or CPPS certification preferred.
  • Experience
    • Minimum 3–5 years of experience in healthcare, required. Preferably clinical quality improvement, clinical operations, population health, compliance or regulatory reporting or healthcare data analysis.
    • Experience with Rural health Clinic quality reporting, ACO quality measures, CMS value-based care programs, and/or preventative care and chronic disease metrics preferred..
    • Strong knowledge of Electronic Health Records (EHRs), clinical reporting tools or dashboards and population health platforms preferred.
