Responsibilities
- Perform comprehensive patient assessments, including vital signs and physical examinations, to evaluate health status and identify urgent needs.
- Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards.
- Utilize EMR (Electronic Medical Record) systems such as Epic or Cerner to document patient care accurately and efficiently.
- Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile dressing changes using aseptic techniques.
- Collaborate with multidisciplinary teams to develop and implement personalized care plans, including discharge planning and case management.
- Conduct triage assessments for urgent care or emergency situations, prioritizing patient needs effectively.
- Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, or working with individuals with developmental disabilities.
- Maintain infection control standards and ensure compliance with HIPAA regulations to protect patient confidentiality.
- Provide education on health coaching topics like nutrition, medication management, pain control, and post-acute care instructions.
- Monitor patients on ventilators or with acute pain management requirements in inpatient or outpatient settings.
Qualifications
- Valid RN or LPN license in good standing; previous experience in critical care, hospital medicine, or emergency room settings is highly preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate medical documentation.
- Experience working with diverse patient populations including geriatrics, pediatrics, neonatal ventilator care, or individuals with disabilities.
- Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques.
- Skilled in performing venipuncture, phlebotomy, injection administration, catheter care and sterile processing procedures.
- Ability to handle complex clinical situations involving acute pain management or infectious disease care confidently.
- Strong communication skills for effective patient assessment and collaboration within multidisciplinary teams.
