About Care Resources
Care Resources operates the innovative PACE program (Program of All-inclusive Care for the Elderly) out of 2 beautiful facilities located in Kentwood and Lowell!! Care Resources provides and coordinates all medical care and social services to eligible participants. We provide all physician and nursing care, prescription drugs, durable medical equipment, dental, eye and foot care, hospital care, medical transportation, home care, emergency, and rehab services in our Clinic, as well as social and recreational activities at the Day Center. To be eligible for the PACE program, seniors must be 55 and older, a resident of Kent County or within the zip codes we serve, able to live safely at home, and meet the nursing facility level of care.

What will you do?
As The RN Clinic Nurse, You Will
- Work under the supervision of the Clinic Manager and serve as a member of the Interdisciplinary Team
- Assess, coordinate, monitor, educate, and provide clinical treatment, supplies and services required to meet the ongoing needs of the participants at the clinic and/or in their homes
- There may be some travel to participant homes in Kent County and the outskirts of surrounding counties
- Participate in the on-call rotation with 5 other RNs. All after hours calls are phone triage only
- Licensed as an RN in the State of Michigan. BSN is preferred.
- Requires a minimum of 2 years of acute care, med/surg, skilled nursing, or clinical office experience.
- Prior experience working with the frail/elderly population is preferred.
