Hollan Group on behalf of Prompt Care Management based in Spring Valley, NY is seeking a Nursing Home Transition and Diversion (NHTD) Waiver Services Coordinator to help Medicaid-eligible seniors and people with physical disabilities receive comprehensive quality services in a community-based setting.
The well-qualified individual has a bachelor’s degree and at least 3 years of experience providing case management or social work services for elderly or disabled individuals, as well as a proven track record in helping individuals navigate care options, access services, and live safely and successfully within the community with supportive care. A degree or certification in social work, psychology, health care management, or related field is preferred.
The ideal candidate is self-directed yet collaborative, with excellent time management and organizational skills, high attention to detail, strong verbal and writing skills, and computer literacy. English proficiency is a must, and additional languages are a plus.
The position is fully remote, however qualified candidates must have access to reliable transportation as this position requires the Service Coordinator to visit participants in their homes at least once every three months in the Capital and Adirondack regions.

Pay: $50,000.00 - $60,000.00 per year
