Functional Medicine NP
Location: Farmington, CT (In-Person, Hybrid, or Remote Considered)
Structure: Independent Contractor (1099)
Schedule: Flexible (Minimum 2 clinical days/week)

This role is designed for a Nurse Practitioner who wants to practice real functional medicine with autonomy, depth, and the ability to build meaningful patient relationships.

Clinical Responsibilities
- Conduct comprehensive new patient visits and follow-ups
- Develop personalized, root-cause-based care plans
- Order and interpret advanced functional labs
- Manage medications, supplements, and lifestyle interventions
- Support patients across metabolic, hormonal, and complex chronic conditions
- Maintain clear, timely documentation within the EHR
- Collaborate with the team as needed to support patient outcomes

Compensation

Requirements
- Active Connecticut APRN license
- Full Practice Authority (FPA)
- Active DEA license
- Minimum 2 years NP experience
