Job Description
Primary Duties and Responsibilities
The Social Worker Assistant provides administrative support, programmatic assistance and patient navigation for patients, the Center for Healthy Living staff, and partner agencies. Support includes patient screening, conducting intake interviews, appointment scheduling, case management and overall coordination of the patient care plan.
Coordinates with clients/patients to ensure they are following up on recommendations by Center for Healthy Living staff and assists with obtaining essential services. Contacts appropriate community resources to meet clients’ needs. Maintains regular contact with clients, facilitating communication between the client, family and Center staff. Ensures information on plan of care is provided to all care providers with patient consent. Depending on department needs, may require commuting between multiple sites/ locations in the community.
Performs administrative support duties which include clerical duties and answering telephone calls from referral services, the public, and community agencies. Projects an image of professionalism in communication, appearance, and conduct. Maintains confidentiality. Acts as Hoag Ambassador to patients, visitors, staff, physicians, vendors, and the community.
Performs other duties as assigned.

Qualifications
Education and Experience
- High School Graduate or equivalent required
- Working knowledge of Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint, plus healthcare database systems preferred
- Associate degree in Social Welfare, Social/Human Services, Sociology other behavioral science preferred
- Organized and able to maintain clear and concise case records required
- Working knowledge of public and private social service agencies and other community resources available to meet clients’ needs required
- Knowledge of basic social service needs of individuals and families and basic factors which support individual and family social and physical well-being such as health care, household management and material needs required
- Principles and techniques of interviewing required to obtain information needed to determine the nature of individual and family problem situations required
- Knowledge of problem behavior patterns, appearance and conditions of clients’ indicative of the need for help required
- Knowledge of methods and techniques to support clients in working self-sufficiency and mutually agreed upon goals required
