We are seeking a compassionate and motivated Physical Therapist (PT) to provide skilled home health therapy services. In this role, you'll evaluate patients, develop personalized treatment plans, and deliver evidence-based interventions that improve strength, balance, mobility, and overall quality of life.
What You'll Do
- Perform comprehensive in-home physical therapy evaluations
- Develop and implement individualized treatment plans
- Improve patient mobility, strength, balance, endurance, and functional independence
- Educate patients and caregivers on home exercise programs and safety techniques
- Collaborate with physicians and interdisciplinary care teams
- Complete timely documentation and monitor patient progress
Qualifications
- Active Texas Physical Therapist (PT) license
- Minimum 1 year of Physical Therapy experience
- Home Health experience preferred
- Strong clinical, communication, and patient education skills
- Valid driver's license and reliable transportation
- Current CPR certification (or ability to obtain)
Preferred
- 2 years of Home Health PT experience
- Experience with geriatric and medically complex patients
- Knowledge of Medicare guidelines and OASIS documentation
- Experience with WellSky/Kinnser or similar EMR systems
- Bilingual (English/Spanish) a plus
