Responsibilities

• Provide high-quality patient care in a skilled nursing facility.

• Participate in ongoing professional development and training programs.

• Engage in continuous learning to enhance skills and knowledge.

• Collaborate with interdisciplinary teams to develop patient care plans.

• Achieve performance targets and contribute to team success.

• Monitor and document patient progress and outcomes regularly.

• Maintain high visibility of work activities and results.

• Communicate effectively with patients and their families.

• Utilize pipelines to assess potential success in patient care.

• Ensure compliance with healthcare regulations and facility policies.

• Participate in quality improvement initiatives within the facility.

• Provide mentorship and support to junior staff as needed.

Experience/Qualifications

• Valid occupational therapy license in the relevant state.

• Eagerness to learn and adapt in a fast-paced environment.

• Experience in skilled nursing facilities or similar healthcare settings.

• Commitment to ongoing professional development and training.

• Ability to set and achieve performance targets independently.

• Strong communication skills for effective patient and family interactions.

• Proven ability to collaborate with interdisciplinary healthcare teams.

• Familiarity with healthcare regulations and compliance standards.

• Ability to monitor and document patient progress accurately.

• Experience in quality improvement initiatives within healthcare settings.
