- Assist licensed nurses with patient assessments, care planning, and treatment implementation

- Assist with vital signs, wound care, treatments, and skilled nursing interventions as ordered
- Observe, document, and report changes in patient condition to physicians and leadership
- Participate in admissions, discharges, and transfers
- Provide clear communication to patients, families, and healthcare team members
- Collaborate with the interdisciplinary care team to ensure comprehensive care
- Participate in care conferences and care plan updates
- Promote teamwork, professionalism, and effective communication
- Assist with state surveys and inspections as needed
Qualifications:
- Graduate of an accredited Licensed Practical Nurse (LPN) program
- Current, unencumbered State of Rhode Island LPN license
- Current CPR certification (ACLS preferred, not required)
- Long-term care or skilled nursing experience preferred (not required)
- Strong clinical judgment, communication, and interpersonal skills
- Ability to handle multiple priorities with professionalism and tact
