Hillcrest Home pays every other Friday through direct deposit.

Physical Therapist Starting @ $50.00 /hr

Physical Therapist Assistant Starting @ $30.00 /hr

-Job Description-

We are seeking a dedicated and compassionate Physical Therapist, and a Physical Therapist Assistant to join our team. The ideal candidate will have a strong understanding of anatomy and physiology, as well as experience in developing and implementing care plans tailored to individual patient needs. You will play a crucial role in assisting patients with activities of daily living and promoting their overall health and well-being.

-Responsibilities-

• Evaluate, diagnose and treat patients to improve mobility, relive pain and manage chronic conditions (PT only)
• Develop customized treatment plans using exercise, manual therapy and equipment while educating patients and documenting progress (PT only)
• Review medical history, monitor treatment efficacy and collaborate with health care teams (PT only)
• Provide supervision for PTAs (PT only)
• Assess patient needs and develop personalized treatment plans based on specific conditions and goals
• Assist patients with ADLs to enhance their independence and quality of life
• Provide therapeutic interventions in various settings, including geriatrics, pediatrics, inpatient, and outpatient care.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
• Utilize medical imaging and other diagnostic tools to inform treatment decisions.
• Maintain accurate documentation of patient progress and treatment outcomes.
• Educate patients and their families on the therapeutic process and strategies for continued improvement.
• Stay current with best practices in occupational health and rehabilitation.

-Job Requirements-

• Physical Therapy Degree (PT only)
• State licensure is mandatory (PT only)
• Associate’s degree from an accredited Physical therapist Assistant Program (PTA only)
• Must pass the National Physical Therapy Exam and meet state-specific licensing or certification requirements (PTA only)
• Strong knowledge of medical terminology, anatomy, physiology, and occupational health principles
• Experience in skilled nursing facilities or similar environments is preferred
• Familiarity with developing care plans for diverse patient populations, including those with acute conditions or chronic illnesses
• Excellent communication skills to effectively interact with patients, families, and interdisciplinary teams
• Ability to demonstrate patience, empathy, and professionalism in all interactions
• Previous experience in home health or clinic settings is a plus
