What You’ll Do
- Evaluate patients and create personalized treatment plans to improve function, mobility, and overall well-being.
- Provide physical therapy services in client homes while ensuring the highest quality of care.
- Collaborate with nurses, occupational therapists, and other members of the interdisciplinary team.
- Accurately document care and communicate progress to ensure seamless coordination.
- Educate patients and families on exercise, safety, and home programs to support lasting results.
