Speech Therapist – PRN – Home Care
Innovate Rehab & Wellness, part of the Hillcrest Health and Living family, is currently seeking a PRN Speech Therapist to join our team providing in home therapy services in the Omaha area.
The Speech Therapist enhances the lives of individuals by utilizing their assessment and evaluative skills to diagnose, treat and help to prevent disorders related to speech, language, cognitive communication, voice, swallowing and fluency.

- Completes evaluations for referred individuals, utilizes appropriate tests and measures, establishing a plan of care in compliance with departmental standards and federal and state regulations.
- Completes patient treatments efficiently in accordance in accordance with plan of care.
- Completes accurate charging of services and provides documentation to support the charges and provision of the therapy services within guidelines.
- Manages each patient’s plan of care effectively by timely interdisciplinary collaboration and communication with physician involvement as needed.
- Effectively plans transition of patients within or outside the Hillcrest system.
- Graduate from an accredited speech therapy program required.
- At least two years of experience performing as a Speech Therapist in a long term care, post-acute, hospital, home care or similar setting preferred.
- Must be licensed in the state of Nebraska or compact state as a Speech Therapist.
