What You’ll Do
- Provide one-on-one OT services in patients’ homes, focusing on safety, function, and independence
- Complete comprehensive assessments and develop individualized treatment plans
- Train patients and families on adaptive equipment, energy conservation, and home safety strategies
- Collaborate with an interdisciplinary team (nursing, PT, social work, aides, etc.) to coordinate care
- Document timely and accurately in the EMR according to regulatory and agency standards
- Participate in case conferences and team meetings as needed
- Current Occupational Therapist (OT) license in the state of practice
- Graduation from an accredited Occupational Therapy program
- Previous home health, SNF, or rehab experience preferred, but strong new grads will be considered
- Valid driver’s license, reliable transportation, and appropriate auto insurance
- Strong communication, organization, and time-management skills
- Compassionate, independent, and comfortable working in a variety of home environments
