Job Summary
We are seeking a dynamic and compassionate Resident Care Coordinator - Nurse to lead and coordinate comprehensive patient care within our long-term care and assisted living facility. Duties
- Coordinate and oversee daily resident care activities, ensuring adherence to nursing policies and state healthcare regulations
- Conduct Quality Assurance Audits
- Develop, implement, and update individualized care plans tailored to each resident’s needs, including dementia care, geriatrics, behavioral health, and memory care
- Collaborate with physicians, therapists, social workers, and family members to facilitate discharge planning, medication management, tube feeding protocols, and other treatments like IV infusion or catheter care
- Supervise nursing staff and care teams by providing guidance on infection control practices, aseptic techniques, medication administration, and emergency procedures such as airway management or ventilator support
- Ensure compliance with HIPAA regulations by maintaining accurate medical documentation and medical records while managing medical coding (ICD-10/ICD-9) for billing and reporting purposes
- Complete resident admissions and discharges
