PROGRAM SUMMARY: The Highland Behavioral Rivers Health Developmental Disabilities program provides activities, services, learning opportunities and supports to help individuals with intellectual and/or developmental disabilities live independently in their communities. The program provides year-round, quality-driven social, recreational, vocational and educational programs for individuals, and provides support to their families.
Duties and Responsibilities include:
- Complete intakes for Family Support Program referrals, providing orientation to families and explanation of services/funding
- Write comprehensive Individual Family Support Plans based on individual’s needs
- Enter new individuals in electronic health record
- Scan documents into electronic health record within 7 days.
- Travel as needed across 13 counties and to out of town provider meetings or training.
- Attend meetings on individuals and community resource fairs, or home visits as requested
- Answer phones and provide quality customer service
- Assist families with other resources and networking as needed, complete surveys
- Process Family Support vouchers
- Enter information into budget spreadsheet
- Complete filing and assure accuracy of charts
- Proofread all work for accuracy
- Work with or assist in transition to other programs with regional office staff, support coordinators, providers, and other community agencies.
- Does not have individuals on a waiting list.
- Attends Monthly staff meetings
- Completes required training timely.
- Any additional duties assigned by supervisor.
