About the Role:
As a Social Services Director at Highland Park Rehabilitation & Care, you will be responsible for:
- Providing crisis intervention and counseling services to residents and their families
- Developing and implementing individualized care plans that address the social and emotional needs of our residents
- Coordinating with our interdisciplinary team to ensure that our residents receive comprehensive care and support
- Conducting social histories and assessments to identify individual needs and develop strategies for addressing those needs
- Maintaining accurate and up-to-date records of resident care and services
- Collaboration with other healthcare professionals to ensure continuity of care and optimal outcomes
- Develop, implement, and evaluate social services programs and services
- Provide crisis intervention, counseling, and support to residents and their families
- Conduct social histories and assessments to identify individual needs and develop strategies for addressing those needs
- Coordinate with other healthcare professionals to ensure continuity of care and optimal outcomes
- Maintain accurate and up-to-date records of resident care and services
- Collaborate with other departments to ensure effective communication and coordination of care
- A minimum Bachelor's degree in Social Work (BSW). However, A Master of Social Work (MSW) is highly preferred.
- Minimum of 1 year experienced as a full-time, supervised social worker in a health care setting working directly with individuals. Experience in geriatrics or skilled nursing facility (SNF) is highly preferred.
- Must hold an active license or certification (e.g., LSW,LCSW)
- Strong interpersonal and communication skills
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
- Ability to maintain confidentiality and handle sensitive and confidential information
- Ability to adapt to changing situations and priorities
- $66,000.00-$80,000.00 - Dependent on Experience/Education
