About the Role:

As a Social Services Director at Highland Park Rehabilitation & Care, you will be responsible for:

• Providing crisis intervention and counseling services to residents and their families
• Developing and implementing individualized care plans that address the social and emotional needs of our residents
• Coordinating with our interdisciplinary team to ensure that our residents receive comprehensive care and support
• Conducting social histories and assessments to identify individual needs and develop strategies for addressing those needs
• Maintaining accurate and up-to-date records of resident care and services
• Collaboration with other healthcare professionals to ensure continuity of care and optimal outcomes
  
  
  

Responsibilities:

• Develop, implement, and evaluate social services programs and services
• Provide crisis intervention, counseling, and support to residents and their families
• Conduct social histories and assessments to identify individual needs and develop strategies for addressing those needs
• Coordinate with other healthcare professionals to ensure continuity of care and optimal outcomes
• Maintain accurate and up-to-date records of resident care and services
• Collaborate with other departments to ensure effective communication and coordination of care
  
  
  

Requirements:

• A minimum Bachelor's degree in Social Work (BSW). However, A Master of Social Work (MSW) is highly preferred.
• Minimum of 1 year experienced as a full-time, supervised social worker in a health care setting working directly with individuals. Experience in geriatrics or skilled nursing facility (SNF) is highly preferred.
• Must hold an active license or certification (e.g., LSW,LCSW)
• Strong interpersonal and communication skills
• Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
• Ability to maintain confidentiality and handle sensitive and confidential information
• Ability to adapt to changing situations and priorities
