Program & Location: Harbour House Family Center - New Bedford, MA
Education & Experience: High School Diploma or GED, preferred. Must have a valid driver's license, vehicle, and is willing to travel.
Pay Range: $20 - $27/hr (education & experience dependent)
Status/Schedule: Full Time; Monday-Friday 8am-4:30pm
*Travel required w/ mileage reimbursement

About Us

SEMCOA's (Southeast Massachusetts Council on Advocacy) mission is to advocate for and provide support to individuals, families, and communities in the areas of substance use, mental illness, domestic violence, homelessness, and parenting. SEMCOA operates four emergency assistance congregate family shelters and additional scattered sites that provide supportive services to families and assist them with securing permanent housing. SEMCOA also provides residential recovery home treatment to individuals with substance use disorders, as well as transitional and permanent housing stability and support programs. Our portfolio also includes two graduate sober houses, the Family Preservation Program, as well as Greater Brockton and Greater Attleboro Taunton Healthy Families programs.   

Case Manager Requirements

  • Must present as courteous and professional at all times;
  • Demonstrated flexibility and ability to perform multiple tasks;
  • Must have excellent communication skills, including the ability to organize ideas in logical and clear fashion;
  • Knowledge of basic computer skills and software programs including the ability to learn, use and train HPAO Electronic Health Record (EHR);
  • Must have the ability to work with others in cooperative and collaborative manner;
  • An understanding of the relationship between substance use and mental health issues and/or self-help programs;
  • Understand and maintain therapeutic boundaries and reinforce rules in a respectful manner;
  • Comply with Federal Regulations, 42 CFR Part 2, Confidentiality of Substance Use Disorder and HIPAA Regulations;
  • TB/Flu and Covid19 Assessment;
  • Completion of the HPAO approved de-escalation program;
  • Successful completion of all trainings upon hire and annually as required. 

Case Manager Duties & Responsibilities

  • Maintain assigned caseload;
  • Providing direct care to residents – including assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily groups facilitation, group note completion, and completion of bio-psychosocial assessments;
  • Making referrals when appropriate;
  • Responsible for each assigned residents’ medical records. All records are to be professional, complete and thorough with entries made in a timely manner;
  • Responsible for linking resident’s with aftercare services prior to discharge and aiding the resident and his/her family access services when appropriate;
  • Strengthen current and create new partnerships within the community to ensure that the clients develop the skills needed to sustain sobriety and/or housing;
  • Review patients/family rights and responsibilities, emergency safety plans and grievance policy
  • Communicating/involving family in treatment at patient/client/resident’s request;
  • Maintain professionalism when connecting with individual’s collaterals;
  • Attend and participate in all unit, facility meetings, case conferences, Treatment Team meetings, and mandatory trainings, as directed by the Clinical Supervisor /Director;
  • Responsible for continued education in the field respective to their service level;
  •  Comply with the McKinney-Vento Homeless Assistance Act, Efforts to Outcomes (ETO) / Homeless Management Information System (HMIS) / CaseWorthy reporting requirements;
  • Have a thorough understanding of program policies, EOHLC’s Scope of Services, review and enforce Program Guidelines and Uniform Shelter Rules with participants
  • Maintain thorough knowledge of public and private resources in the community;
  • Understand and follow established referral system for necessary services to appropriate agencies/providers including but not limited to Department of Transitional Assistance (DTA), Department of Children and Families (DCF), school systems, Medical/Mental Health Services;
  • Assist participants with the development of Assessment Based Re-Housing Plan and evaluate progress of stated goals and action steps;
  • Assist families with HomeBASE application, relocation benefits, and Strategic Rehousing Initiative (SRI) funds;
  • Meet with participants for a minimum of once per week to evaluate progress identifying potential barriers to establishing/ maintaining housing and develop subsequent strategies to resolve;
  • Conduct entrance and exit interviews, assessment of families; development of assessment-based rehousing plan, stabilization plans as required;
  • Assist families in developing economic stability plans, maximizing income, & obtaining employment to establish long-term housing strategies with participants;
  • Assist with transportation/childcare arrangements to enable parents to attend meetings related to the Assessment Based Re-Housing Plan;
  • Work and communicate with team members  on family’s progress;
  • Welcome families, schedule events and meetings for families upon entering shelter and facilitating workshops for participants on assigned topics;
  • All other duties as assigned by Program Director / Clinical Supervisor/ COO.

Case Manager Qualifications

  • Associate's degree in human services, High School Diploma, or GED preferred.
  • One year of experience in substance use treatment and/or mental health field.
  • CPR Certification preferred.
  • Personal lived experience in mental health and/or substance use recovery.
  • Experience working with families with children.
  • Valid Driver’s License, vehicle, and able to provide clean driving record.
  • Spanish or Haitian Creole speakers encouraged to apply.
