With programs located throughout Southeastern Massachusetts, High Point provides a full continuum of care — including inpatient, outpatient, residential, and community-based services.

Requirements

  • Must always present as courteous and professional; 
  • Maintains and submits any current Licensure or Certificate necessary for the performance of the position;
  • Demonstrated flexibility and ability to perform multiple tasks; 
  • Must have excellent communication skills, including the ability to organize ideas in logical and

clear fashion; 

  • Knowledge of basic computer skills and software programs including the ability to learn, use and

train HPAO Electronic Health Record (EHR); 

  • Must have the ability to work with others in a cooperative and collaborative manner; 
  • An understanding of the relationship between substance use and mental health issues and/or self-

help programs; 

  • Understand and maintain therapeutic boundaries; 
  • Reinforce rules in a respectful manner; 
  • Comply with Federal Regulations, 42 CFR Part 2, Confidentiality of Substance Use Disorder and

HIPAA Regulations; 

  • TB/Flu and Covid19 Assessment; 
  • Annual Completion of the HPAO crisis de-escalation program; 
  • CPR Certification; 
  • Successful completion of all trainings upon hire and annually as required. 

Duties and Responsibilities

  • Crisis intervention;
  • Maintain assigned caseload; Providing direct care to patients/residents – including assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily groups facilitation, group note completion, and completion of bio-psychosocial assessments;
  • Making referrals when appropriate;
  • Responsible for each assigned patients/residents’ medical records. All records are to be professional, complete and thorough with entries made in a timely manner;
  • Responsible connecting patients/residents with aftercare services prior to discharge and aiding the resident and his/her family access services when appropriate;
  • Communicating/involving family in treatment at patients/residents’ request;
  • Attend and participate in all unit, facility meetings, case conferences, Treatment Team meetings, and mandatory trainings, as directed by the Clinical Supervisor /Director;
  • Responsible for continued education in the field of substance use and related courses.
  • Always utilize standards precautions;
  • Demonstrates behaviors that recognize the rights of patients as defined by the patient’s
  • rights;

Residential Services

  • Adherence, to insurance regulations, including requesting extensions after 90 days of treatment if needed;
  • Supporting patients in crisis through verbal de-escalation, 1:1 monitoring, or crisis/safety planning, as requested;
  • Participation in program initiatives, quality projects, and program support services.
  • Facilitation of identified groups and milieu activities as assigned.

Inpatient Services

  • Responsible for contacting the insurance company for authorization, as needed;
  • Utilize walkie-talkie as campus responders;
  • Respond to Codes as necessary;
  • Coordination of aftercare services, appointments and referrals.
  • Facilitation of identified groups and milieu activities as assigned.
  • Participation in MDT meetings to support patient care coordination;
  • Assistance with monitoring patient observation checks, as needed;
  • Completion of required documentation, including: consents/releases, Group Notes, Progress Notes, Contacts Logs, Transition/Discharge Summaries.
  • Assistance with patient observation checks for monitoring milieu safety, as requested;
  • Supporting patients in crisis through verbal de-escalation, 1:1 monitoring, or crisis/safety planning, as requested;
  • Attendance in supervision, staff trainings, and MDT meetings;

Outpatient Services (Clinic, MCI Services)

  • Assistance with monitoring patient safety while in the program including waiting rooms, restrooms, boarding room (MCI Only), hallways.
  • Screening of patient walk ins or phone calls for services to the Masters Clinician;
  • Care coordination activities to ensure referrals for services are completed
  • Monitoring consents & releases are completed upon admission and as needed thereafter.
  • Accompany MA Clinician during evaluation in the community (MCI only).
  • Completion of assigned follow up sessions with patient/family post evaluation as scheduled (MCI Only).
  • Assist with scheduling appointments for follow-up with nurse case manager, clinician and other services requested.
  • Attendance at MDT meetings and documenting MDT notes for patients, as requested
  • Participation in program initiatives, quality projects, and program support services.
  • Coverage of phones/reception area as needed.
  • Engagement in training opportunities to enhance knowledge of mental health and SUD services.
  • Assignment of coordinator responsibilities, as designated by supervisor.
  • Engagement and attendance at community events with stakeholders, agencies and service providers.
  • Care coordination with external agencies, stakeholders and referral sources to provide patient progress reports, compliance reports, other requested documentation at the request of the Masters Clinician.

Community-Based Services (RSN/CSN/CSP-JI)

  • Engagement with patients in community settings and inpatient units to offer services;
  • Completion of initiation for services and all required documentation based upon services agreed upon;
  • Ongoing monitoring and compliance with required documentation for services.
  • Monitoring service authorizations and providing updated information for continued stay reviews;
  • Engagement and completion of Recovery Coach Training;
  • Care coordination activities to ensure referrals for services are completed
  • Monitoring consents & releases are completed upon admission and as needed thereafter.
  • Assist with scheduling appointments for follow up with designated community providers
  • Participation in program initiatives, quality projects, and program support services.
  • Engagement in training opportunities to enhance knowledge of mental health and SUD services.
  • Engagement and attendance at community events with stakeholders, agencies and service providers.

Youth Point

  • Submission of all required driving documentation per policy at hire and annually thereafter (TM);
  • Transport and engagement with youth in community settings (TM);
  • Completion of all CBHI New Hire Trainings upon hire (TM);
  • Possesses an understanding of trauma informed care and cultural competence and attends all additional trainings as required/assigned;
  • Ability to run psycho educational and social support groups as needed;
  • Knowledge of area resources necessary to meet treatment plan goals of youth;
  • Ability to network and develop professional working relationships with external area providers;
  • Ongoing regular communication and collaboration with other internal and external providers;
  • Completion of all required documentation at the start of services and updates as required by performance specifications;
  • Ability to accurately document time spent with clients, guardians, and collaterals including interventions utilized and responses to these interventions;
  • Ongoing monitoring and compliance with required documentation for services;
  • Tracking service authorizations dates/units and providing updated information for continued stay reviews, as necessary for the service provided;
  • Comply with all necessary medical necessity criteria and performance specifications, dependent on service provided;
  • Other duties as assigned by the Clinical Director or Program Director

Salary Range: $18 - $20

Any compensation range provided is an estimate based on current market data. The actual offer may fall above or below this range, depending on the candidate’s experience, skills, and geographic location.
