Clinical Liaison, LVN – Hospice (Dual Role) | San Diego, CA
Join High Care Hospice during an exciting time of growth. We’re seeking a relationship-driven LVN to support referrals, EOBs, admissions, and clinical operations across the San Diego area.
This dual-role position blends business development clinical impact, ideal for someone with strong community connections who is passionate about end-of-life care.
What you’ll do:
- Build and manage referral relationships (hospitals, SNFs, MDs)
- Generate hospice referrals and drive census growth
- Complete EOBs and hospice admissions
- Support clinical coordination and operations
Who you are:
- LVN licensed in CA (RN/other nursing license considered)
- Hospice/home health experience required
- Strong network in San Diego is a plus
- Compassionate, proactive, and growth-minded
