Job Description

The Registered Professional Nurse IV, Care Manager is responsible for providing outcome-oriented resident/patient care with an appropriate length of stay, use appropriate resources based on specific case types, promote the integration and coordination of clinical services, monitor the use of patient care resources, support collaborative practice and continuity of care, and enhances resident/patient, provider and employee satisfaction. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his or her age specific needs.

POSITION SUMMARY

Care Management Services

  • The RPN IV (Care Manager) coordinates a variety of activities (care management and discharge planning services) to expedite the care plan and assure that patients’/residents’ needs are met within insurance parameters and serves as a point of contact on a rotating basis, making daily work assignment, coverage for leaves, advise on state and federal regulatory requirements and other resources as appropriate.
  • Performs initial assessment of need for case management services, identification of discharge needs through the evaluation of the patient’s social, psychological, cultural and family situation.
  • Uses the principles of growth and development to assess each patient’s age specific needs and provide age specific treatment and care.
  • Uses the principles of pain management in assisting each patient to be as comfortable and pain free as possible by assessing, treating and educating patient and family.
  • May rotate as a Care Management resource/charge person for the RPN V (Patient Services Nurse Manager), making work assignments, coverage for leaves, advises on state and federal regulatory requirements, etc.

Discharge Planning

  • Presents and explains IM from Medicare/Discharge Appeal letters to patients.
  • Facilitates patient/resident/family assessments.
  • Participates in the level of care determination.
  • Works cooperatively as a member of the interdisciplinary treatment team.
  • Coordinates care with community groups, establishes contacts and referrals for community resources to plan for continuity of care.
  • May assist and/or arrange for transportation upon discharge.
  • May assist and/or arrange for transfers from facility to facility.
  • Initiates court involvement, as appropriate, to assure appropriate services from a patient advocate perspective.
  • May assist and/or arrange for substance abuse treatment as appropriate.
  • Completes chart notes/assessments when service is provided, as ordered.
  • Assists physician in facilitating post-hospital care.


MINIMUM QUALIFICATIONS:
To qualify, you must meet all of the following requirements. Please note that unless specifically indicated, the required education and experience may not be gained concurrently. In addition, qualifying work experience is credited based on a 40-hour workweek.

Education: Graduated from an accredited School of Nursing. While not required, graduation from an accredited nursing program with a Bachelor’s Degree from an accredited college or university is highly desired.

Experience: One and one half (1 ½) years of professional nursing work experience in a hospital, clinic or institution. Certified Professional in Utilization Review or Certified Case Management Certification preferred. Utilization management and case management experience preferred.

Supervisory Aptitude: is the demonstration of aptitude or potential for the performance of supervisory duties through successful completion of regular or special assignments which involve some supervisory responsibilities or aspects; by serving as a group or team leader; previous work experience in a position that demonstrates or performs supervisory functions or in similar work in which opportunities for demonstrating supervisory or administrative capabilities exist; by completion of training courses in supervision accompanied by application of supervisory skills in work assignments; or by favorable appraisals by a supervisor indicating the possession of supervisory potential.

Substitutions Allowed: Possession of a master's degree from an accredited college or university school of nursing may be substituted for one year of nursing experience.

QUALITY OF EXPERIENCE: Possession of the required number of years of experience will not in itself be accepted as proof of qualification for a position. The applicant's overall experience must have been of such scope and level of responsibility as to conclusively demonstrate that he/she has the ability to perform the duties of the position for which he/she is being considered.

Licensure: License to practice as a Registered Nurse (RN) in the State of Hawaii.

Certification: BLS certification required within one (1) month of appointment.

  • Rates listed for paid holidays, vacation and sick days are for 1.0 Full Time Equivalent (FTE) positions assigned to 8 hour shifts. Accruals for these benefits are otherwise prorated based on FTE.

Pre-Employment Health Requirements

Offers of employment will be contingent on successfully passing a pre-employment examination, which includes a drug screen and other regulatory medical requirements such as, but not limited to, a tuberculosis (TB) screen. The cost for drug screening and blood draw for QuantiFERON and Titers, shall be borne by the Hawaii Health Systems Corporation.
