Part-Time Physical Therapist – Home Health

Job Summary
We are seeking a compassionate, licensed Physical Therapist to join our growing home health team. In this role, you will provide one-on-one physical therapy services to patients in their homes, helping them regain mobility, improve function, and enhance their quality of life.

Responsibilities
- Perform comprehensive physical therapy evaluations.
- Develop individualized treatment plans based on patient needs and physician orders.
- Provide skilled therapeutic interventions in patients' homes.
- Monitor patient progress and modify treatment plans as appropriate.
- Educate patients and caregivers on exercises, safety, and home programs.
- Complete accurate and timely clinical documentation.
- Communicate effectively with physicians, nurses, and the interdisciplinary care team.
- Maintain compliance with all home health regulations and company policies.
Qualifications
- Current Physical Therapist (PT) license in the state of practice.
- Graduate of an accredited Physical Therapy program.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Home health experience preferred but not required.
- Strong communication, organization, and time management skills.
