Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately following physician orders, including IV infusions and pain management protocols.
- Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile dressing changes using aseptic techniques.
- Monitor patients’ conditions closely, including those with acute pain, infectious diseases, dementia or Alzheimer's care needs, and those requiring ventilator or respiratory support.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet long-term care regulations and best practices in geriatrics and memory care.
- Provide patient education on medication administration, disease management (including ICD-9/ICD-10 coding), and health coaching tailored to diverse populations including pediatric, neonatal, or geriatric patients.
- Maintain accurate medical documentation in compliance with HIPAA standards while ensuring clinical confidentiality policies are upheld at all times.
Skills
- Proven experience working with Electronic Health Records (EHR) systems such as Epic, Cerner, or Athenahealth for documentation and clinical data management.
- Strong background in critical care environments including ICU or Level I Trauma Center experience; familiarity with trauma protocols is highly desirable.
- Expertise in managing complex cases involving acute care scenarios like emergency medicine, urgent care settings, or hospital medicine.
- Knowledge of advanced nursing procedures such as venipuncture (phlebotomy), specimen collection/processing, airway management, and cardiac catheterization support.
- Skilled in patient assessment techniques for diverse populations including geriatrics, pediatrics, neonatal patients or individuals with disabilities.
- Ability to communicate effectively with patients and caregivers about treatment plans while demonstrating empathy and professionalism.
- Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), CPT coding for procedures, and documentation standards aligned with NCQA standards.
- Certification in Basic Life Support (BLS), CPR, First Aid, and other relevant credentials required for safe practice across multiple healthcare environments.
