Job Summary
We are seeking a dynamic and compassionate Licensed Practical Nurse (LPN) to step into the vital role of Charge Nurse within our healthcare team. Duties
- Provide direct patient care, including assessments, vital signs, medication administration, and wound care, ensuring safety and comfort.
- Supervise nursing staff on your shift, delegating tasks effectively and offering guidance to promote best practices in patient care.
- Coordinate interdisciplinary communication for comprehensive treatment planning, discharge procedures, and ongoing care needs.
- Maintain accurate medical documentation using electronic health record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA and long-term care regulations.
- Oversee clinical procedures including IV infusions, catheterization, sterile processing, and airway management to uphold safety standards.
- Conduct patient assessments for diverse populations including geriatric patients with dementia or Alzheimer’s disease, individuals with developmental disabilities, or those requiring memory or behavioral health care.
- Lead emergency response efforts utilizing skills in first aid, CPR, acute pain management, trauma protocols (Level I & II trauma experience), and infection control measures.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice.
- Proven experience working in various healthcare settings such as hospitals (including ICU or emergency medicine), skilled nursing facilities, assisted living communities, or home & community care environments.
- Demonstrated expertise in critical care scenarios including ICU experience, trauma center procedures, infectious disease care, and acute care management.
- Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth; proficiency in ICD-9/10 coding and medical documentation standards.
- Strong knowledge of physiology, anatomy, medication administration protocols—including injections—and medical terminology.
- Experience working with diverse patient populations such as pediatric (PICU), neonatal ventilator management, or adult geriatrics requiring dementia or Alzheimer’s care.
- Excellent caregiver communication skills with the ability to assess patient needs accurately and collaborate effectively with multidisciplinary teams.
- Additional qualifications preferred include triage experience, hospice or palliative care familiarity, tube feeding management, ventilator support knowledge, and experience working with individuals with disabilities or behavioral health needs.
