What are the responsibilities and job description for the Blended Service Coordinator position at Heritage Valley Health System?
To act as the single point of accountability for adults with serious mental illness and children with mental illness or emotional disturbances. This is achieved through assessment, service planning and coordination of care, based on the identified wants and needs of those served. Services are individualized and focus on the “whole” person in recovery.
Requirements
Bachelors Degree in Psychology, Social Work, Counseling or related field with three years experience working directly with individuals in MH/ID area.
Valid Pennsylvania Drivers license.
Successful completion of applicable clearances as outlined in Human Resources policy HR-106 prior to commencing employment.
Preferred
Master’s Degree and case management experience. Extensive knowledge of persons with disabilities, knowledge of regulatory agencies and payer policies, as well as community resources.
