Lead Care Manager (ECM) – Hybrid Role

This is a hybrid position (2 days remote, 3 days in the field), giving you the flexibility to work from home while building strong, in-person connections with members across High Desert regions.
Key Responsibilities

Serve as the primary point of contact for members and coordinate their care

Develop and manage personalized care plans with members and providers

Collaborate with healthcare teams and community resources

Support members through education, advocacy, and engagement

Identify and address gaps in care
- Maintain accurate, timely documentation
Requirements

Experience in care or case management (community-based preferred)

Knowledge of Medi-Cal and ECM strongly preferred

Strong communication and interpersonal skills

Ability to work in a team-oriented environment

Willingness to travel within Orange County
- Bilingual (English/Spanish) preferred
