Your Impact as an ACT Community Support Specialist


As a Community Support Specialist, you’ll work directly with adults experiencing significant mental health challenges, providing intensive, recovery-focused services where they live and spend their time.


Rather than waiting for people to come to us, ACT brings support into the community — helping individuals navigate real-life challenges while building confidence, stability, and independence.


You’ll work as part of a highly collaborative multidisciplinary team that may include therapists, nurses, prescribers, substance use specialists, and peer support professionals — all working together toward shared recovery goals.

What You’ll Do


Support Recovery in Real Life



  • Provide community-based case management and support services

  • Meet with individuals in homes, healthcare settings, and community environments

  • Build trusting relationships and help individuals achieve personal recovery goals

  • Support engagement in treatment and ongoing wellness


Help Individuals Build Skills & Independence



  • Teach coping and symptom-management strategies

  • Assist with daily living skills including budgeting, time management, meal preparation, and personal care

  • Provide medication education and support

  • Encourage meaningful community involvement and positive social supports


Connect People to Resources



  • Assist individuals with housing stability and retention

  • Help clients access benefits such as Medicaid, SNAP, and SSI/SSDI

  • Coordinate care with medical, behavioral health, dental, and community providers, (including by not limited to providing transportation to clients in your personal vehicle)

  • Advocate for services and supports that promote long-term success


Collaborate as Part of a Team



  • Participate in ACT team meetings and treatment planning

  • Work closely with clinicians, healthcare providers, probation, DCFS, and community partners

  • Maintain timely and accurate documentation

  • Contribute to a team-centered approach to care

  • Participation in providing rotating on-call coverage for clients in the ACT program at least 3 times in a 4 week period.

Qualifications


Required



  • High School Diploma or GED (With 5 years’ experience working in a mental health setting preferred)

  • Minimum age of 21

  • Valid driver’s license, reliable transportation, and proof of automobile insurance

  • Experience working with individuals experiencing mental health and/or substance use concerns


Preferred



  • Bachelor’s degree in social work, psychology, education, or related human services field

  • Experience in behavioral health, case management, crisis support, or community services




Schedule & Compensation


Full-Time | Monday–Friday | 8:00 AM – 5:00 PM


Every Other Friday Off – Paid Wellness Day


Rotating on-call coverage for clients in the ACT program at least 3 times in a 4-week period.


 


Starting Pay


High School Diploma/GED: Starting at $18.00/hour


High School Diploma/GED 5 years qualifying experience: Starting at $23.00/hour
Bachelor’s Degree: Starting at $25.00/hour


Compensation is based on education, experience, licensure, and certifications.
