About the Role:

The Home Health LVN (Licensed Vocational Nurse) plays a crucial role in providing high-quality, compassionate care to patients in their homes. This position involves assessing patient needs, developing care plans, and delivering nursing care under the supervision of a registered nurse or physician. The LVN will monitor patient progress, administer medications, and educate patients and their families about health management. By fostering a supportive and safe environment, the Home Health LVN ensures that patients receive the necessary care to maintain their health and independence. Ultimately, this role contributes significantly to improving the quality of life for patients and their families in the community.

Minimum Qualifications:

• Current LVN license in the state of practice.
• Completion of an accredited vocational nursing program.
• Basic Life Support (BLS) certification.

Preferred Qualifications:

• Experience in home health care or a similar setting.
• Bilingual skills to communicate effectively with diverse patient populations.
• Wound Care Certification

Responsibilities:

• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and document patient progress, reporting any changes to the supervising nurse.
• Provide education and support to patients and their families regarding health management.
• Assist with daily living activities, ensuring patient comfort and safety.

Skills:

The required skills for this position include strong clinical assessment abilities, which are essential for evaluating patient needs and developing effective care plans. Communication skills are vital, as the Home Health LVN must interact with patients, families, and healthcare teams to provide education and support. Time management skills are necessary to prioritize tasks and ensure that all patient care responsibilities are met efficiently. Additionally, problem-solving skills are important for addressing any challenges that may arise in patient care. Preferred skills, such as bilingual communication, enhance the ability to connect with a broader range of patients, ensuring that all individuals receive the care and support they need.
